How to Request an SMMC Rescreening After Something Changes

What to have in front of you

• Prior screening date, rank, and case information
• Hospital or emergency discharge papers
• New diagnosis, accident, fall, or functional change
• New or expected living arrangement
• Date and details of caregiver loss or reduced availability
• Photos or records of home damage or unsafe conditions when relevant

What to say when someone answers

“They were screened for SMMC LTC on [date]. I am reporting a significant change: [plain description]. This changes their ability to perform [tasks] and the help available because [reason]. I am requesting a rescreening and written confirmation of the next step.”

Small but useful: Write down the person’s name, the date, the exact next step, and when you should call again.

Do this in order

   Name the qualifying change and the date it occurred or is expected.

   Connect it to daily tasks and safety, not only a new diagnosis.

   Explain what unpaid help was lost or cannot continue.

   Ask when the rescreening will occur, what evidence to send, and how to get the new screening and rank in writing.

What happens next

ADRC staff repeat the phone screening and the system generates a new priority score and rank based on current circumstances. A rescreening does not guarantee waitlist release or Medicaid eligibility.
