What to Do While You Are on Florida’s SMMC Long-Term Care Pre-Enrollment List
Release from pre-enrollment is based on priority and frailty—not simply how long the person has waited.

Keep the ADRC updated, save the written priority rank, and request a rescreening whenever health, living arrangements, the home, or caregiver support changes. In Brevard, call 407-514-0019 and ask for interim programs rather than waiting for one program to solve every need.
What to have in front of you
- The written priority-rank notice and screening date
- Any new discharge paperwork, diagnoses, falls, or emergency visits
- Changes in bathing, dressing, toileting, walking, eating, memory, or supervision needs
- Changes in caregiver health, availability, work, or relationship
- Current address, phone, and authorized-representative information
What to say when someone answers
“They are already screened for SMMC LTC. I need to confirm the current rank and contact information, report changes, and ask which interim programs can address respite, meals, transportation, or help at home now.”
Small but useful: Write down the person’s name, the date, the exact next step, and when you should call again.
Do this in order

Request and keep a copy of the 701S screening and priority score.

Confirm the ADRC has the correct mailing address, phone, and authorized representative.

Report significant changes when they occur; do not wait for the annual rescreening.

Apply separately for interim programs. Medicare home health, caregiver respite, meals, transportation, and utility help use different eligibility rules.
What happens next
Florida groups screening scores into frailty-based ranks. Higher-priority people are released as enrollment becomes available and then move through clinical and financial eligibility steps.
If they say “we’ll put you on the list”
Ask three questions: What is the current rank? What change would justify rescreening? Which program can address the most urgent need while SMMC is pending? Put the follow-up date on a calendar.
