How to Apply for Florida Medicaid Long-Term Care in Brevard
The first task is an ADRC screening—not immediately completing a Medicaid application.

Start by calling the Brevard-area Elder Helpline at 407-514-0019 and requesting a Statewide Medicaid Managed Care Long-Term Care screening. Florida says this phone screening usually takes about 45 minutes to one hour and produces the priority score used for pre-enrollment placement.
What to have in front of you
- Daily tasks the person cannot do safely without help
- Recent falls, hospital visits, diagnoses, or changes in memory and behavior
- The caregiver’s availability, health limits, job demands, and whether that help may end
- Current living arrangement and any expected move
- Names of insurance plans and approximate finances for later eligibility steps
What to say when someone answers
“I want to start the SMMC Long-Term Care process. Please schedule the 701S phone screening and tell me what information you need to score current needs and caregiver availability accurately.”
Small but useful: Write down the person’s name, the date, the exact next step, and when you should call again.
Do this in order

Complete the ADRC phone screening before trying to choose an LTC managed-care plan.

Answer based on what happens on a difficult normal day and what would happen if the caregiver were unavailable.

Ask for a copy of the completed screening tool and written priority rank.

Keep contact information current and respond quickly to letters. DCF determines financial Medicaid eligibility later; the screening does not do that.
What happens next
A high priority rank can lead to placement on the SMMC LTC pre-enrollment list. A low rank does not go on the pre-enrollment list, but the notice should include community-resource information and instructions for a hearing or rescreening.
If they say “we’ll put you on the list”
Ask for interim programs such as caregiver respite, meals, transportation, ADI, HCE, or Community Care for the Elderly. If health, housing, or caregiver support changes, request a rescreening right away.
