How to Request Crisis Enrollment From the APD Pre-Enrollment List
How to document an immediate APD crisis, request a determination, and protect appeal rights if the request is denied.

Contact the person’s APD regional office or assigned APD contact and clearly say you are requesting a crisis determination under the current APD rules. Describe the immediate event, why existing supports cannot keep the person safe, and what is likely to happen without urgent services.
What to have in front of you
- The APD application or pre-enrollment information
- A one-page timeline of the crisis and why it is happening now
- Hospital, police, abuse-report, eviction, caregiver-health, or provider records
- A list of supports tried and why they are not enough
- Names and phone numbers for people who can verify the facts
What to say when someone answers
“I am requesting an APD crisis determination for [name]. The immediate risk is [specific event]. Current supports cannot prevent [likely harm or loss of placement]. Please tell me what crisis documentation is required, who is deciding, and when I will receive a written decision.”
Small but useful: Write down the person’s name, the date, the exact next step, and when you should call again.
Do this in order

Call APD and identify the exact crisis category or rule being considered.

Send a short factual packet connecting each record to the immediate safety or placement risk.

Ask for confirmation that the request is complete and the expected decision date.

Keep the written decision and follow its appeal instructions immediately if denied.
What happens next
APD reviews the facts under its crisis criteria and may ask for additional records. A crisis determination can affect priority for waiver enrollment, but it is not automatic simply because care is difficult or a diagnosis is severe.
If they say “we’ll put you on the list”
If APD treats the request as routine, repeat that you are asking for a formal crisis determination and written decision. Call Disability Rights Florida at 1-800-342-0823 if a deadline, safety issue, or appeal is unclear. Call 911 for immediate danger.
