• Primary-care clinic or medical-center name
• Caregiver name, relationship, phone, and email
• Current caregiver program or application status
• The immediate need: PCAFC, PGCSS, respite, training, home care, or a reported change

What to say when someone answers

“The Veteran receives care through [facility or health system]. I need the correct Florida Caregiver Support Program team for [need]. Please give me the team’s direct contact and confirm who owns the follow-up.”

Small but useful: Write down the person’s name, the date, the exact next step, and when you should call again.

Do this in order

   Identify the VA health system shown in the Veteran’s appointments or care-team information.

   Choose Florida in the official caregiver-team directory and match the health system.

   Leave one concise message with the Veteran’s facility, caregiver contact, and requested program or service.

   If no response, call the national Caregiver Support Line and ask it to confirm the correct team and escalation route.

What happens next

The local team can explain PCAFC and PGCSS, provide caregiver support, and connect the family with the Veteran’s clinical team or social worker for home and community services.
