What VA Home-Based Care Can Help With
VA home care is a group of programs—not one benefit—and the right starting point depends on whether the need is medical, personal care, respite, or control over a care budget.

Ask the Veteran’s VA primary-care team or social worker for a home-and-community-based-services assessment. VA options can include Home Based Primary Care, skilled home health, homemaker/home health aide care, respite, adult day health, telehealth, palliative care, hospice, and Veteran-Directed Care where available.
What to have in front of you
- Veteran’s VA health-care and primary-care information
- Medical tasks needed at home: nursing, therapy, medication, monitoring, or wound care
- Daily personal-care and supervision needs
- Caregiver availability, health, and need for respite
- Current Medicare, Medicaid, hospice, community-care, or private services
What to say when someone answers
“I need a broad VA home-care assessment. Please separate the medical needs, personal-care needs, caregiver respite, and long-term support, then tell us which programs are available through this VA system.”
Small but useful: Write down the person’s name, the date, the exact next step, and when you should call again.
Do this in order

List medical and personal-care needs separately so the team can match more than one program.

Ask whether Home Based Primary Care fits complex ongoing medical needs that are hard to manage through clinic visits.

Ask separately about skilled home health, homemaker/home health aide, respite, adult day health, and Veteran-Directed Care.

Get each referral, authorization owner, provider, start date, possible copay, and backup plan in writing.
What happens next
The VA team assesses clinical need and local availability, then may combine services. Home Based Primary Care uses an interdisciplinary VA team; other services may be delivered by VA or authorized community providers.
If they say “we’ll put you on the list”
Ask which need can be addressed first and whether a different program can cover the gap. Request reassessment after hospitalization, falls, caregiver loss, or a major change in daily function.
