Call Sunshine Line at 813-272-7272. Staff will screen the resident and identify the correct application for medical, social-service, grocery, wellness, or other eligible trips.

- Social Security number requested by the program
- Disability and available transportation information
- Household income information

Steps in order

Complete the phone screening.

Request the correct application and checklist.
- 3
Keep proof of submission and eligibility.
- 4
Once enrolled, reserve at least two days ahead and schedule the return trip.
If the first answer is no or wait
Ask whether HARTPlus, a bus pass, Medicaid transportation, or another referral fits if door-to-door eligibility is not approved.
