VA Forms 10-305 vs. 10-306: Which Caregiver Form Do You Need?

VA Form 10-305 explains your options after a Program of Comprehensive Assistance for Family Caregivers decision and is included with current PCAFC decision letters. VA Form 10-306 is the form you submit when you need information about a previous PCAFC decision, such as the decision date or a copy of the letter. Neither form is the appeal itself.
What to have in front of you
- Every PCAFC decision letter you have received
- The Veteran’s name and VA information used on the caregiver application
- Approximate decision dates if a letter is missing
- The envelope or notice date for any current appeal deadline
- A copy of anything mailed to VA and proof of delivery
What to say when someone answers
“I am trying to choose the correct PCAFC form. Do I only need information or a copy of an earlier decision, or am I trying to challenge the decision? Please confirm the form, deadline, and correct submission address.”
Small but useful: Write down the person’s name, the date, the exact next step, and when you should call again.
Do this in order

Use Form 10-305 to understand the review and appeal choices listed for a PCAFC decision.

Use Form 10-306 only to request information about a previous PCAFC decision, including the date or a copy of the decision letter.

If you disagree with the decision, choose the actual review or appeal form identified by VA; Form 10-305 describes the options but does not start the appeal.

Mail a completed Form 10-306 to the VA Evidence Intake Center address printed on the current form or VA caregiver appeals page, and keep proof.
What happens next
A Form 10-306 request should lead VA to provide the requested PCAFC decision information. A review or appeal follows a separate process and deadline based on the option selected.
If they say “we’ll put you on the list”
Call the Caregiver Support Line before a deadline expires. Ask which stage the request is in and whether VA needs another form or document. Do not file a duplicate appeal simply because a records request is pending.
