What to Do When Social Security Starts a Disability Review
How to answer a Continuing Disability Review without losing benefits because of missing forms or incomplete records.

Open the notice immediately and return the requested review form by the deadline. Update Social Security on every treating source and explain what still limits daily activity and work, even when the diagnosis has not changed.
What to have in front of you
- The notice, bill, decision, or discharge paperwork connected to the problem
- The person’s full name, date of birth, address, insurance information, and identification
- A short timeline with dates, names, phone calls, and what changed
- Copies of medical, school, financial, or care records that directly support the request
What to say when someone answers
“I received a Continuing Disability Review notice. I want to confirm which form is required, the deadline, and whether Social Security has my current providers.”
Small but useful: Write down the person’s name, the date, the exact next step, and when you should call again.
Do this in order

Calendar the due date and submit the short or long review form exactly as instructed.

List treatment from the review period, including hospitals, therapists, tests, medication changes, and unsuccessful work attempts.

Explain current limitations and any help needed with daily tasks; do not write only “same as before.”

Respond promptly to Florida DDS calls, record requests, or examination appointments.
What happens next
Social Security reviews the form, may send the case to Florida DDS, and then mails a written continuation or cessation decision with appeal rights.
If they say “we’ll put you on the list”
If you cannot obtain a record, tell DDS where it is and document your attempts. Never ignore the form because a doctor promised to send records.
