AdventHealth says patients may apply through the AdventHealth app or call 855-241-2455 for application help. Confirm the approval period and participating providers because outside clinicians may not be covered.
What to have in front of you
- Account numbers, facilities, and dates of service
- Household size and income proof
- Insurance information and Medicaid-screening communications
- Tax return, W-2, pay records, or support letter if requested
- Separate statements from outside providers
What to say when someone answers
“I need financial assistance for Florida AdventHealth services. Please tell me how to apply, what proof is required, and which hospital and physician bills the decision will cover.”
Small but useful: Write down the person’s name, the date, the exact next step, and when you should call again.
