How to Complete a Florida DNRO Form
The yellow form, required signatures, copies, placement, and why a living will is not enough for EMS.

Use Florida Form DH 1896 on yellow paper and complete it with an authorized Florida clinician. The patient or authorized representative and clinician must sign; keep it visible and available to emergency responders.
What to have in front of you
- The notice, bill, decision, or discharge paperwork connected to the problem
- The person’s full name, date of birth, address, insurance information, and identification
- A short timeline with dates, names, phone calls, and what changed
- Copies of medical, school, financial, or care records that directly support the request
What to say when someone answers
“We want to discuss and complete Florida Form DH 1896. Who may sign, what color paper is required, and where should copies be kept so EMS can honor it?”
Small but useful: Write down the person’s name, the date, the exact next step, and when you should call again.
Do this in order

Discuss resuscitation goals with the treating physician, osteopathic physician, autonomous APRN, or physician assistant authorized by Florida rules.

Print the current form on yellow paper and complete both required signature sections.

Keep a visible copy at home, carry the wallet device, and send a copy with the patient to facilities or appointments.

Tell family, caregivers, hospice, home-health staff, and facility staff where the form is located.
What happens next
The clinician documents the order. EMS can honor the properly completed Florida form or device when it is presented or observed.
If they say “we’ll put you on the list”
If a facility uses its own DNR order, ask whether the Florida yellow DNRO is also needed for transport and home. Replace damaged or outdated copies promptly.
