How to Register for Brevard’s Special Needs Shelter Program
The local application, medical information, transportation request, annual update, and what to bring.

Register before a storm if a Brevard resident has medical needs that cannot be supported in a regular shelter and has no safer alternative. Request evacuation transportation on the same application when needed.
What to have in front of you
- The notice, bill, decision, or discharge paperwork connected to the problem
- The person’s full name, date of birth, address, insurance information, and identification
- A short timeline with dates, names, phone calls, and what changed
- Copies of medical, school, financial, or care records that directly support the request
What to say when someone answers
“I need to register a Brevard resident for the Special Needs program and request transportation. What medical documentation and annual update are required?”
Small but useful: Write down the person’s name, the date, the exact next step, and when you should call again.
Do this in order

Complete the online or paper Special Needs application with current medical, equipment, medication, caregiver, and contact information.

Include transportation needs, mobility limits, oxygen, electricity-dependent devices, and pet information.

Ask the medical provider, home-health agency, hospice, or equipment company to help with required clinical details.

Confirm registration and update the record every year or whenever needs, address, equipment, or contacts change.
What happens next
Emergency Management reviews the application and uses the registry for shelter and transportation planning. Registration does not mean a shelter is open or that every medical need can be accommodated.
If they say “we’ll put you on the list”
If approval is pending during a storm, call Emergency Management and 211. Do not travel to a shelter until officials announce that it is open and tell you where to go.
