How to Apply for CHAMPVA as a Spouse or Survivor
Confirm CHAMPVA eligibility first, then submit one complete application with every applicable insurance document.

Use VA’s online CHAMPVA application or VA Form 10-10d. CHAMPVA may cover an eligible spouse, surviving spouse, or child of a qualifying Veteran or service member, but a person eligible for TRICARE cannot receive CHAMPVA. If the applicant has Medicare or other health insurance, include the required cards and VA Form 10-7959c when directed.
What to have in front of you
- Applicant and Veteran sponsor identifying information
- Front and back of Medicare and other health-insurance cards; Part D card if applicable
- Marriage certificate, civil-union certificate, or common-law marriage affidavit to speed spouse processing
- Relevant divorce, annulment, or death records for a surviving spouse when applicable
- VA Form 10-7959c Other Health Insurance Certification when required
- Proof of Medicare ineligibility for an applicant age 65 or older who is not enrolled
What to say when someone answers
“I am applying for CHAMPVA as a [spouse or surviving spouse]. Please confirm the sponsor eligibility, whether TRICARE affects eligibility, and every supporting document and insurance form required for my situation.”
Small but useful: Write down the person’s name, the date, the exact next step, and when you should call again.
Do this in order

Check the sponsor and applicant eligibility rules before completing the packet.

Apply online or use the current VA Form 10-10d. List each person applying and the Veteran sponsor accurately.

Upload or mail clear copies of every applicable insurance, Medicare, relationship, and status document. For mailed double-sided records, copy both sides onto scannable pages.

Save the online confirmation or a complete copy and tracking proof for the mailed packet.
What happens next
VA reviews sponsor and dependent eligibility and contacts the applicant if it needs more information. Approved applicants receive a CHAMPVA ID card and program guide by mail. Coverage coordination depends on Medicare and other insurance.
If the first answer doesn’t solve it
Do not send a duplicate application merely because processing is pending. Call the CHAMPVA number with the submission date and ask whether the application is in the system or a missing-document letter was issued.
