How to Appeal When Medicare Home Health Services Are Ending
Act from the written notice immediately; fast-appeal deadlines can be much shorter than ordinary Medicare appeals.

Read the service-ending notice as soon as it arrives and call the appeal organization listed on it by the printed deadline if you want a fast review. Ask the home-health agency for the records and clinical reason it says covered care should end.
What to have in front of you
- The complete service-ending notice and date received
- Medicare card and plan information
- Current home-health plan of care and visit schedule
- Recent nursing, therapy, wound, and functional notes
- A clinician statement explaining continued skilled need and homebound status
What to say when someone answers
“I am requesting a fast appeal of the decision to end home-health services on [date]. Please confirm the deadline, case number, coverage during review, and how my records and clinician statement should be submitted.”
Small but useful: Write down the person’s name, the date, the exact next step, and when you should call again.
Do this in order

Call the independent review contact printed on the notice before its deadline.

Tell the agency and treating clinician an appeal is pending and request supporting records immediately.

Explain the skilled care still needed and the likely effect of stopping now.

Keep every notice and follow the next appeal level if the first decision is unfavorable.
What happens next
An independent reviewer examines whether Medicare-covered services should continue and issues a decision under the fast timeline. A Medicare Advantage plan may have additional procedures stated in its notice.
If they say “we’ll put you on the list”
If no notice was provided or the deadline already passed, call Medicare and the plan or agency immediately and ask about all remaining appeal and complaint options. Do not assume a verbal discharge ends every right.
