How to Request Miami-Dade Elderly Care Planning for Someone Living Alone
Turn a general worry about someone living alone into a documented list of safety risks, daily needs, and referral follow-ups.
Start here
Call Community Action and Human Services at 786-469-4600 and ask about elderly care planning. If aging services are also needed, request an Alliance for Aging screening at 305-670-4357.
Ask for elderly care planning
See county elderly servicesWhat to have ready
- The person’s address, phone, age, and preferred language
- Specific concerns: falls, food, utilities, memory, medicines, isolation, or exploitation
- Who has permission to speak with agencies
- Names and contact information for current providers or trusted contacts
Words you can use
“I am concerned about an older adult living alone in Miami-Dade. The specific risks are [list]. I need care-planning help and a screening for services, not just a general resource list.”
Steps in order
- 1
Separate immediate danger from longer-term planning; call 911 for immediate danger.
- 2
Ask what consent or authorization the person must give.
- 3
Create one list of referrals, owners, deadlines, and follow-up dates.
- 4
Ask about meals, home help, transportation, emergency planning, and caregiver support as separate needs.
If the first answer is no or wait
If the person refuses help and has decision-making capacity, ask what low-pressure options remain. If abuse, neglect, or exploitation is suspected, use Florida’s Abuse Hotline rather than relying only on care planning.
