Steps in order

Use the TD form rather than the ADA form unless both may apply.

Keep a copy and submission date.

Ask how approval, denial, fees, and ride booking are handled.
If the first answer is no or wait
Ask about ADA Passport if disability prevents fixed-route use. LeeTran is not Lee County’s Medicaid transportation provider; Medicaid members should check their plan’s ride benefit.
